HOW TO ORDER PPE THROUGH EMERGENCY MANAGEMENT:
1.) Review WA DOH PPE Prioritization of Allocation Document and determine your eligibility
2.) Fill Out the PPE Order Form
3.) Fill our a WA 213 RR Form
4.) Email the PPE Order Form and WA 213 RR Form to: EOC-LOGS@graysharbor.us
*Please note, orders are filled weekly and have to be prioritized based on needs and resources. There may be an extended period of time between submitting and receiving an order.
